
Foundations + was established in March of 2000 through funding from MCSS. It is a transition service available to young adults who have been diagnosed with a developmental disability. In 2005 the Foundations program was encompassed by a new program called Passports to Community. Foundations + was available to youth aged 16-21 years. Passports to Community will still be available to youth at age 16 but young adults will be involved in the program for a longer period of time to ensure a successful transition from school to community.
Young adults who have been diagnosed with a developmental disability can be referred to our Foundations/Passports program by Developmental Services. When leaving school our Agency supports young adults to make connections in their community, set attainable goals and identify resources that will bring them closer to their personal dreams. Areas relating to community living which are addressed include employment, volunteer, social/leadership, recreation/leisure and self-help skills. We also share information about continuing education and ways to establish and maintain friendships.
Foundations/Passports is committed to supporting people to make informed choices about future life directions that have been established through Transition Plans and Life Planning.
To help achieve goals that were set by participants, Foundations+/Passports applied for and has received a grant from the Ontario Trillium Foundation to create a business called "The Lite Lunch Express". There are presently 6 people with developmental disabilities cooking lunches such as soup, salad, sandwiches and wraps and delivering them to people that work in Barry's Bay who have pre-ordered them. Participants are learning many employable skills such as cooking, packaging, team work and planning as well as making many community connections when delivering lunches.
